ALL chaperones must submit a background check form.
Magnolia ISD requires all chaperones to have a background check done before they will be allowed
to chaperone. The required forms are available in the band director's office. It can also be printed from the following website:
The completed forms need to be returned to Mr. Voltz or the MHS front office.
